Short-Term Student Missions Church Assistance Request Form

SUBJECT: Spring, Summer, and Fall Youth Missions Program
AGE CATEGORY: All college age, youth group, and members of the
congregation
MISSION
STATEMENT: To
encourage mission mindedness in our youth and provide them with experiences
that can help them gain an expanded worldview of Christianity.
MISSION
OBJECTIVE: To
provide Christian leadership, encouragement, guidance and financial support to
our youth in order to further the mission programs at the Kerrville Church of
Christ.
Applicants,
please complete the following:
1. List your full name, including address, phone number, and email
address.
2. Date of mission trip and the projected cost. (The missions
committee needs enough lead time to procure what types of funds are necessary
to support said trip.)
3. Brief description of mission trip. (Be sure to include location,
kind of transportation, any groups that are involved in this mission work.)
4. Who is the coordinator of the mission team/group? (List any phone numbers you have next to
their name.)
5. Write briefly of your background.
Include such things as any church activities you participated at your
home congregation and what church you are attending right now. List a few of your personal goals.
6. What amount of the total funds can you raise through your
immediate family and friends?
7. Will there be other financial support coming in from other congregations? If so, how much?
After
you complete the first page of this request form, please collect three personal
recommendations from outside your own family (i.e. elders, ministers,
teachers). Attach these recommendations
along with the first page and mail to:

Summer
Break Missions: April 1
Winter
Break Missions: October 1
Spring
Break Missions: January 15
After
your request form has been processed and necessary criteria has been met, then a
short interview will be arranged with Marilyn Knight.
We
request that you take photos and keep a journal of your mission experiences so
you will be able to give a brief report to our congregation when you return
home.
